Provide case management support.
Assess client's needs or presenting issues, where necessary provide timely interventions and actions for desired outcomes.
Participate in case management meetings.
Collaborate closely with various departments and agencies to meet the psycho-social emotional needs of clients to ensure a safe home or social environment for holistic care.
Coordination of AIC or MSF or social referrals for financial or psychosocial care services.
Support Medical Escort Transport team and Home Personal Care services for clients.
Application for financial grants, FA case report and financial counselling.
Provide home environment safety, hygiene, well-being supportive care and interventions.
Support community outreach.
Assist in social documentation.
