Job Responsibilities

• Plan and perform outreach programs and interventions
• Conceptualize, propose and implement programs and activities to reach out or attract target groups
• Engage and manage volunteers in conducting programs and mentoring youths
• Cultivate a support network and build working relationships with relevant agencies and volunteers to ensure success of programs
• Facilitate invention work for youths identified to be at-risk
• Build trust and instill positive values among youths
• Engage, mentor and counsel alienated and at-risk youths who reject formal activities so as to build rapport for future intervention
• Develop and maintain a trusting relationship with youths
• Build and engage social networks to identify opportunities for social services targeting youths in particular
• Create opportunities for mentoring, coaching and supporting youths to facilities their personal, social and educational growth
• Keep proper records, tracking, monitoring and evaluation systems
• To provide feedback to Social Workers on progress of residents in their therapeutic goals and liaise closely with Care Staff team regarding program schedules
• To ensure that the residents are punctual and on time for activities and programs

Requirements

• Strong team-player, with natural ability to interact with patients/ residents, healthcare team and community partners of all levels.
• Organised, analytical, able to fit different pieces of the puzzle together.
