Key Responsibilities
Clinical Assessment & Care Planning
• Conduct comprehensive admission and periodic assessments using standardizedtools (e.g., Modified FRAT, FAST).
• Develop, implement, and update individualized care plans via the Elder &Community Caresuite.
• Perform critical reassessments following hospitalizations, falls, orsignificant absences.

Medication & Clinical Management
• Oversee medication management, including preparation, packing, andadministration.
• Ensure strict adherence to the IDDSI framework for residents with swallowingdifficulties (dysphagia).
• Respond promptly to medical emergencies and maintain rigorous infectioncontrol standards.

Leadership & Supervision
• Supervise and train care staff in delegated nursing procedures, enteralfeeding, and medication support.
• Liaise with the Centre Lead regarding nursing equipment and day-careoperations.
• Lead quality improvement initiatives to enhance resident safety andwell-being.

Coordination & Communication
• Collaborate with a multidisciplinary team to ensure seamless care coordination.
• Maintain effective clinical communication with families regarding clientconditions and care goals.
• Facilitate internal and external referrals to support holistic residentneeds.
