As part of the multi-disciplinary team with the National University Health System (NUHS)@Home, you will provide quality pharmaceutical care as our Pharmacist.

Job Responsibilities

* Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions

* Medication counselling on transfer medications by a variety of means (in-person and virtual)

* Medication reconciliation

* Review discharge prescriptions appropriately

* Verify new orders by the physicians and identify inappropriate medication/doses if relevant

* Assist with perform medication supply duty

* Follow up on medication preparation status

* Undertake duties which may be assigned by the NUHS staff member depending on the requirements

Requirements

* Bachelor of Science in Pharmacy from an accredited college by Singapore Pharmacy Council

* At least 2 years of relevant experience

* Fully registered with the Singapore Pharmacy Council

* Possess excellent communication, interpersonal and organisation skills

* Able to work independently and in a team

* Undertake duties which may be assigned by the NUHS staff member depending on the requirement

* Weekdays and/or weekends working hours are available
