Responsibilities

• Conduct home visits for acute and chronic medical cases

• Perform clinical assessments, diagnoses, and treatment plans

• Provide medical advice, medication review, and follow-up care

• Work closely with our clinical team to coordinate patient management

• Ensure accurate documentation and compliance with MOH guidelines

Requirements

• Fully registered with Singapore Medical Council (SMC)

• Valid Practising Certificate

• Experience in primary care, family medicine, acute care or GP setting preferred

• Comfortable with home-visit environment and patient interaction

• Professional, empathetic and confident in independent decision-making

• Able to commit to either scheduled shifts or flexible hours (depending on role type)
