Job Description :

• Create personalised care plans tailored to the needs of the resident
• Implement care plans and regularly record residents' progress
• Design and organise programs and activities to address residents' care requirements
• Prepare and submit review reports and monthly interview records every 6 month
• Collaborate with relevant stakeholders to coordinate and ensure comprehensive care for residents

Job Requirement :

• Min Diploma in Social Work/Psychology
• At least 2 year of relevant healthcare industry experience
• Working Hours: Mondays to Fridays, shift work with DO
