Work location: Jurong West Daycare Centre

Job purpose

• To provide optimal level of care in accordance to established policies and procedures.
• To assist in activities and programs promoting healthy aging for clients.
• Supervise Nursing Team comprising of Enrolled Nurse, Nursing Aides and Healthcare Assistant to ensure optimal delivery of required care to the clients.

Duties and responsibilities:

• Comply with approved Nursing SOPs
• To adhere to infection control and educating family and staff.
• Work with teams to analyze and evaluate individualized care plans (inteRai)  and ensure care plans are implemented and share with team.
• Identify symptoms of clients related to illness or decline in functional/cognitive abilities and advocate early attention.
• Mentor and guide ENs, NAs and HCAs in performing their functional duties
• Plan monthly roster and ensure safety of the environment.
• Administer medications and treatments as prescribed and update accurately in the cares system.
• Assist clients to manage commonage-related conditions such as incontinence and loss of mobility.
• Coordinate with family on all matters
• Other additional operational duties as assigned by Centre Manager

Qualifications & Skills Required:

• Diploma or degree in nursing
• Registered with SNB
• 3 years clinical experience in eldercare sector will be advantage
• Dynamic team player with good interpersonal communication and presentation skills
