Job Description

SAMH SAY-IT! (Sports, Arts, Youth Integrated Team) provides assessment and intervention for youths and engages youths for ongoing monitoring and support. YIT also works actively with youth agencies, schools and community partners to provide holistic case management for youths and their families.

Job Purpose
The Senior Case Worker/ Social Worker supports Head in overseeing the YIT team to ensure service delivery is in accordance to the intended programme objectives and care model for YIT programme as outlined under the terms & conditions of the Letter of Award/Addendum. This role requires the identified personnel to have the relevant competencies, with sufficient years of direct work experience and service with clients with mental health issues, necessary for the purpose of supervision of staff in the respective field.

He/She assists in maintaining an overview of the service planning and implementation of the YIT programme. He/She will work closely and provide guidance to the YIT team to achieve the YIT goals of outreach to increase mental health awareness, promote early identification and link those at risk and with needs to the appropriate support and resources, and provide follow up support.

In addition, the Senior Case Worker/ Social Worker will establish and/or maintain networking with other community partners or agencies, for purpose of collaboration and facilitate care coordination.

Job Responsibilities
A). Programme Development and Implementation

  • Plan, implement and monitor (together with Team Lead) the outreach, care process, referral and follow-up workflow.
  • Supports Head in supervising the team to provide care and support, in alignment with the overall YIT programme objectives, adhering to the relevant ethical and legal guidelines.
  • Provide care coordination and service linkage to clients with or who are at risk of mental health issues, with the appropriate resources and support including caregiver support.
  • Responsible for establishing the network with other stakeholders like community partners and/or related agencies to build collaborative partnerships and better facilitate care and support for clients and caregivers.
  • Supports Head in establishing and reviewing work and communications workflow including identification, referral and escalation work processes as well as the development/update of the YIT service manual.

B). Administration and Staff Development

  • Assists in ensuring programme data and documents are kept and maintained, in accordance to the YIT programme requirement, as well as the prevailing personal data protection and security guidelines.
  • Supports Head in the training and professional development of the YIT staff team with relevant competencies to provide the appropriate care and support for clients and caregivers, aligning to the intended care model of YIT. This includes ensuring all new/existing staffs are clear on their job scope and responsibilities.
  • Ensure that the team practices are in adherence to the prevailing organisational, ethical, safety and legal requirements, where relevant.
  • To attend monthly supervision with the Head and to coordinate and/or provide supervision to YIT staff team

C). Other Duties

  • Participate in relevant meetings with key stakeholders including AIC to share key learning points where required.
  • To carry out any other duties assigned by the Head and Chief Executive Officer.

    Qualifications & Experience
  • Minimum Degree in Psychology or Social Work.
  • Minimum 3 to 4 years of relevant work experience.
  • Experience working in Social Service Agencies especially youth work, will be an added advantage.
  • Experience with facilitation, case management and counselling.
  • Must be proficient in MS Word, Excel and Powerpoint.
  • Experience with and having skills in the arts (visual arts, music, drama, dance, etc) and/or sports will be an added advantage.
