Job Description
Job Role
- Assist in running of targeted outreach and cognitive stimulation activities
- Support initial assessment of walk-ins & referral cases, follow-up with intervention plans for clients as per the prescribed care plan and under guidance from the Team Lead or Assistant Team Lead (in Team Lead's absence)
- Support in monitoring the Clients’ conditions and progress, and escalate to Team Lead or Assistant Team Lead
Requirement
- Diploma in relevant field (e.g. social work, psychology, counselling)
- Basic computer skills (MS Office – Excel, Outlook, Powerpoint and Word)
- Read & write emails
Work Experience
- Has minimum of 2 years of healthcare/ community care experience

- Working hours are 9am - 5pm
- Staff might have to help out after working hours and weekends at times
