Job Description

Job Summary

The incumbent will assist with Home Personal Care work and apply intervention strategies appropriately, under the guidance of the HPC Care Manager, to resolve presenting issues and improve the well-being of the clients. He/she will assist the Care Manager with the management of case referrals and coordination with HCAs in the provision of services.

Responsibilities

  • Assist the HPC Care Manager to achieve key performance targets and monitor progress indicators in compliance with regulatory requirements
  • Manage case referrals and coordinate with Health Care Assistants (HCAs) to deliver quality Home Personal Care services
  • Guide HCAs in performing their functional duties to ensure effective service delivery
  • Collaborate with PEACE-Connect centre-based and home-based services to integrate care efforts
  • Support the HPC Care Manager in preparing and submitting regulatory reports accurately and on time to relevant authorities
  • Coordinate with care partners to implement care interventions including medication reminders, wound care management, CGT, NGT, urine catheterization, and activities of daily living (ADL) support
  • Educate caregivers and clients on health issues to enhance their understanding and promote appropriate care practices
  • Maintain accurate and up-to-date documentation of client cases to support continuity of care
  • Identify service gaps and communicate findings to the HPC Care Manager for timely follow-up and resolution
  • Perform additional duties as assigned by the Reporting Office


Job Requirements

  • GCE 'O' Levels / Diploma
  • 2 years of relevant experience
  • Passion to work with seniors; good knowledge of eldercare and social services; good organizational skills, team-oriented with excellent interpersonal and communication skills; resourceful and collaborative, ability to converse in various languages and dialects is an advantage
