The Community Case Management Service (CCMS) supports seniors and their caregivers who face challenges managing multiple health and social care needs. Our goal is to empower seniors to manage care at home and to age in place for as long as possible.

As an Assistant Senior / Senior Social Worker in Allkin CCMS, you will play a key role in delivering holistic case management, supporting cross-sector collaboration, and guiding junior practitioners. You will work closely with a multidisciplinary team to serve clients and caregivers with professionalism, compassion, and clinical excellence.

RESPONSIBILITIES

Clinical Work

• Conduct triage and intake assessments for members of the community.
• Perform comprehensive needs and risk assessments to support holistic case management for individuals and families.
• Conduct home visits to assess clients' needs and develop personalized care plans.
• Perform psycho-social assessments and recommend appropriate services to support socio-health integration for clients and caregivers.
• Collaborate with internal departments and community partners to develop, implement, and monitor case plans for seamless care coordination.
• Maintain accurate, timely, and organized case documentation.
• Participate actively in internal and external training, supervision, team meetings, and networking with community partners.
• Facilitate liaison with partner agencies and conduct supervised sessions under the Supervised Access Programme for Vulnerable Adults.

Professional Leadership & Supervision

• Uphold and promote adherence to ethical codes and social work practice standards within the team.
• Review highrisk and/or complex cases and provide recommendations to ensure quality of care.
• Provide supervision to social workers and offer consultation to colleagues and community partners on highrisk cases.
• Support the ongoing development of clinical competencies within the department.
• Provide field supervision and share professional knowledge with students in social work, counselling, or gerontology programmes.

• Bachelor's, Postgraduate Diploma, or Master's Degree in Social Work.
• Passion for working with seniors and caregivers in a community-based setting.
• Strong ability to work independently and collaboratively as part of a multidisciplinary team.
• Strong communication, interpersonal, and organizational skills.
• Proficiency in written and spoken English.
• Fluency in a Mother Tongue language and/or dialects is an advantage.
• Commitment to the Code of Social Work Practice and professional standards.
• Prior experience in supervision.
• 4-6 years of social work experience, with demonstrated competence in casework, counselling, and community work.
