About the Role
We are looking for an experienced nurse to take on a leadership role in delivering high-quality, home-based care for clients with complex medical and functional needs.
This role combines hands-on clinical care with team supervision and care coordination, supporting clients in the community including those with chronic conditions and end-of-life care needs.
You will play a key role in ensuring safe, person-centred care while mentoring nursing staff and strengthening service delivery.
Key Responsibilities
Clinical Care & Service Delivery
Provide direct nursing care for clients in home-based settings
Support care planning and clinical decision-making for complex cases
Monitor care outcomes to ensure safety, quality and continuity
Maintain accurate and compliant clinical documentation
Team Leadership & Supervision
Support supervision of nursing staff and daily operations
Provide on-the-ground guidance, coaching and mentorship
Assist in onboarding, training and competency development
Escalate clinical risks and operational issues where required
Care Coordination & Partnerships
Work with multidisciplinary teams and community partners
Participate in case discussions and care planning
Support care transitions and continuity across services
Quality, Safety & Governance
Support audits, quality improvement and service evaluation
Manage incident reporting and follow-up actions
Ensure compliance with clinical and regulatory standards
Requirements
Bachelor’s Degree in Nursing or equivalent
Registered Nurse with Singapore Nursing Board (SNB)
4–6 years of nursing experience (community / eldercare preferred)
Prior supervisory or senior staff nurse experience is an advantage
Key Competencies
Strong clinical foundation in community or home care
Ability to mentor and guide nursing staff
Good risk assessment and escalation capability
Strong communication and documentation skills
Collaborative and patient-centred approach
